Whenever you gain life, put two +1/+1 counters on Karlov. , Remove six +1/+1 counters from Karlov: Exile target creature.

The scout paraphrase checks out exactly (commander_oracle_ok = true). Two things the printed text makes load-bearing:

  1. Karlov triggers on each lifegain EVENT, not on the amount. Gaining 1 life six separate times = 12 counters; gaining 6 life once = only 2 counters. So the deck is built around frequency of small lifegain triggers (Soul Warden effects, extort, lifelink chip damage, ETB drains) rather than big single heals.
  2. The exile costs six counters = three lifegain events. That’s repeatable hard removal that dodges indestructible/regeneration and hits anything, but every activation shrinks Karlov, so you balance “grow the beater” vs “press the removal button.”

Gameplan

Drop Karlov early, then turn a cheap Orzhov lifegain shell into a counter engine. Every creature you play with a Soul Warden online is a Karlov trigger; every spell with an extort body out is another; every lifelink point is another. Karlov balloons into an evasive-adjacent beater while you bank enough triggers to fire the exile ability on the scariest thing each cycle. Drain payoffs (Marauding Blight-Priest, Epicure of Blood, Vampire Spawn, Kalastria Healer) convert the same lifegain into a clock on all three opponents at once.

Key synergy lines

  • Soul Warden + token/creature flood: Soul Warden / Soul’s Attendant / Suture Priest / Impassioned Orator / Lifecreed Duo / Dazzling Angel / Hinterland Sanctifier / Lunarch Veteran / Anointer Priest each gain 1 per creature ETB. With Doomed Traveler, Hunted Witness, Raise the Alarm, Sacred Cat (embalm), each body is a Karlov trigger — multiple per turn.
  • Extort = a Karlov trigger per spell: Tithe Drinker, Kingpin’s Pet, Basilica Screecher, Syndic of Tithes, Basilica Guards. Every spell you cast lets you pay to gain 1 (and drain 1). In a counterspell-light commons grind, extort is the most reliable repeated trigger you have.
  • Repeatable lifegain mana sinks: Fountain of Youth (: gain 1) and Campfire (: gain 2) turn excess mana directly into Karlov counters every turn — pure inevitability.
  • Lifegain payoffs double-dip: Celestial Unicorn (counter), Marauding Blight-Priest + Epicure of Blood (drain all opponents on every gain), Vanguard Seraph (surveil), Nirkana Assassin (deathtouch on gain). Several removal spells (Moment of Craving, Bake into a Pie’s Food, Faith’s Fetters’ 4 life) are themselves Karlov triggers.

Win condition

Primary: A counter-stacked Karlov as a lifelink-supported beater while the exile ability removes every blocker/threat that matters. Secondary / inevitability: Marauding Blight-Priest + Epicure of Blood drain the table on every lifegain trigger, and Pestilence / Crypt Rats provide a repeatable damage-to-all-faces engine that closes even through a stalled board. You grind opponents out via repeatable exile + life-total pressure, not draw-go.

Role breakdown (counts)

  • Lands — 36: 22 basics (11 Plains / 11 Swamp) + 14 nonbasics. Several gain life on ETB (Scoured Barrens, Fisk Tower, Foot Headquarters, Radiant Fountain, Glimmerpost) = bonus Karlov triggers; Forlorn Flats pings; Bojuka Bog is graveyard hate.
  • Ramp — 9: Arcane Signet, Orzhov Signet, Mind Stone, Prismatic Lens, Guardian Idol, Manakin, Ornithopter of Paradise, Wayfarer’s Bauble, Sylvok Lifestaff (also a lifegain enabler).
  • Card advantage — 9: Read the Bones, Sign in Blood, Night’s Whisper, Costly Plunder, Deadly Dispute, Village Rites, Phyrexian Rager, Gravedigger, Unearth.

Pilot notes

  • Sequence lifegain triggers AFTER Karlov is on the battlefield — a Soul Warden that resolved while Karlov was in the command zone gives no counters.
  • Bank to six and hold exile mana up against the table’s scariest creature; you often want to exile on an opponent’s turn (e.g., a swinging threat) rather than tapping out on yours.
  • Extort on EVERY spell when you can afford the — those single life points are the engine, not chip value.
  • Against go-wide boards, lead with a pseudo-wipe; Eyeblight Massacre and Drown in Sorrow leave most of your small lifegain bodies dead too, so prefer them when you’re behind and use Pestilence/Crypt Rats when you can ping selectively.
  • Fountain of Youth / Campfire are your mana-flood insurance — dump excess mana into Karlov counters end-of-turn.

Weaknesses

  • No counterspells / light stack interaction (commons WB). You’re reactive only through removal; fast combo can outrace you.
  • Karlov is a removal magnet. If he eats exile/bounce you lose the counter bank entirely — lean on the drain payoffs and Pestilence as a backup win that doesn’t need him.
  • Lifegain hate (e.g., Tainted Remedy effects, though rare at a commons table) shuts the engine off.
  • Trigger frequency depends on board presence; after a wipe you rebuild slowly, so don’t over-commit into open mana.
